Health and Wellness

Cindi Lada, our school nurse, is on school/parish campus during the entire school day. She is a liaison between educational personnel and the child’s physician. It is essential that the parents/guardians keep Cindi informed about the child’s medical condition. Health records are kept confidential and locked in the clinic.

Immunization Requirements
Pupils in preschool through grade 8 unless otherwise exempt, must be immunized against certain diseases by type of vaccine and number or required doses.

The parents/guardians submit written evidence obtained from the physician that their child meets or exceeds the minimum immunization requirements, which are:

DPT (diphtheria, tetanus, pertussis) and/or Td Vaccine – dosage: 4 (Although a fifth dose is required if the fourth dose was administered prior to the fourth birthday).

Polio – dosage: 3 (Although a fourth dose is required if the third dose was administered before the fourth birthday).

MMR (Measles, Mumps, Rubella) – dosage: 2 (MMR vaccine should be given at 15 months of age or older. If MMR was given before this age, the child must be revaccinated).

HIB (Haemophilus Influenza b) - dosage: 1 (Effective March 1, 1993 the state is requiring all preschool students to be immunized with the HIB vaccine. Children who receive HIB before 15 months usually receive 4 doses. Children older that 15 months need only receive one vaccine.)

Hepatitis B – dosage: 3 (All children entering school shall be required to provide evidence of having received a three-dose series of Hep B vaccine. The second dose must be admin-istered at least 28 days after the first dose. The third dose must be administered at least two months after the second dose and at least four months after the first dose and at least six months of age.)

Varicella – dosages: must be administered on or after the 1st birthday before beginning kindergarten. A written statement from physician claiming history of disease is acceptable.
